Our registration packets are available through your physician.
You are ultimately responsible for the payment of all charges incurred at the Surgery Center of Kalamazoo. As a courtesy, your insurance benefits are reviewed and verified before your surgical procedure. All charges directly related to your surgery will be billed to your insurance company and we will send you a statement after receiving the plan’s determinations of the covered amount. Please note the bill you receive from the Surgery Center of Kalamazoo will be in addition to any charges or bills you receive from your surgeon or your anesthesia provider.
If your insurance plan requires payment of co-pay and/or deductible, we may request that you pay this amount prior to obtaining services, depending on the type of procedure you are undergoing. Many insurance companies also require pre-authorization or pre-certification before your surgery. Please check with your insurance provider about these requirements prior to surgery. If you do not obtain health plan approval, all charges incurred during your surgery
could become your responsibility. Patients may pay any out-of-pocket expenses by check, cash, money order, or credit card. Surgery Center of Kalamazoo accepts Visa, Master Card, Discover and American Express.
